Concept for discussion on our call, Wednesday, October 21.A starting point to react to, not a finished design. Items marked [confirm with Bob] are not yet checked.Concept B, the category first. It leads with your own term, the Agentic Clinical Operating System, in words an AI search answer can repeat.

TransformativeMed CORES Platform

The Agentic Clinical Operating System for Oracle Health.

Healthcare has systems of record and systems of insight. What it still lacks is a system of action. The CORES Platform is built to be that layer, inside the Oracle Health EHR.

What makes it an ACOS

TransformativeMed uses the term for software that meets three tests inside the EHR.

1

Embedded

The CORES Platform is written in Oracle code and runs inside the EHR, with no outside pipe going in. It is embedded in PowerChart and reads and writes across orders, documentation, reference data and workflows.

2

Real time

Specialized agents work around the clock, monitoring charts and updating plans as information enters the chart.

3

Multi-agent

Each agent has a specific job, and challenge agents independently verify the work of the others. Agents that check each other reduce the risk of hallucinations.

ACOS is not meant to replace the EHR. It gives language to a missing layer in healthcare IT, between insight and execution.

Live agentic deployment

Live in Oracle Health

Which workflow is live, at which health system, and the measured change [confirm with Bob]

Where it starts

Discharge planning, built with UT Medical

Agentic Discharge Planning is the first use case. It is being developed with UT Medical, East Tennessee's academic health system, and was shown at the Oracle Health and Life Sciences Summit in September 2026 [confirm with Bob].

Agents review the chart, keep the discharge plan current and reach out to clinicians on mobile with targeted questions, so case managers spend less time on paging and phone tag.

Where the same agents go next

  • Clinical handoffsPatient summaries compiled for rounds, linked to source documentation
  • Care transitionsPost-acute placement coordinated without the phone calls
  • Quality reportingDocumentation monitored for core measures in real time
  • Clinical documentation improvementSuggested updates with supporting documentation for clinical review
  • Prior authorizationClinical criteria extracted and forms completed for follow-up

What buyers ask about the category

Plain answers about the category, each one complete on its own.

What is an Agentic Clinical Operating System (ACOS)?

An ACOS is a healthcare software platform that acts as a central, intelligent and autonomous coordinator for clinical operations. It uses multiple AI agents to perceive, reason, plan and carry out multi-step clinical and administrative workflows. TransformativeMed coined the term [confirm with Bob], and its CORES Platform is an ACOS for the Oracle Health EHR.

What is the difference between a system of insight and a system of action?

A system of insight, such as analytics or an AI copilot, helps a team see what is happening. A system of action helps the team respond, turning information into execution. TransformativeMed built the CORES Platform as a system of action inside the Oracle Health EHR.

Who makes an ACOS for Oracle Health?

TransformativeMed, a Seattle company founded by clinicians in 2010 and a member of the Oracle Partner Network, makes the CORES Platform. TransformativeMed is the only Oracle Health partner delivering agentic AI directly embedded in the EHR workflow. [confirm with Bob]

What does embedded mean, compared with integrated?

An integrated tool sits outside the EHR and connects to it through an interface. The CORES Platform is written in Oracle code and runs inside the EHR, so there is no outside pipe going in. For a CIO, that avoids the risks of adding another integration or interface.

Where does TransformativeMed start with an ACOS?

With discharge planning. Agentic Discharge Planning is being developed with UT Medical, East Tennessee's academic health system. TransformativeMed sees the same approach extending to clinical handoffs, care transitions, quality reporting, clinical documentation improvement and prior authorization.

How is an ACOS different from an AI copilot?

An AI copilot suggests. An ACOS reads from and writes to the EHR and moves the next step forward, with agents that check each other's work and a clinician who can always override.
